Korean DRG ? Classification Point of View

Abstract
The purpose of the study is to investigate the current status of Korean DRG system from classification point of view. Classification is defined as an arrangement of objects or concepts bases on their essential characteristics. DRG is a joint classification of diagnosis classification, usually International Classification of Disease from WHO, and intervention classification. However, the essential characteristics of classification include clinical homogeneity, economical homogeneity and statistical stability. These characteristics are totally different from those of usual classifications in the
other medical domain. That means, the DRG classification is subject to differentiate by the healthcare systems of adopting countries. Hence, the Korean DRG was reviewed in specific issues such as (1) clinical homogeneity related issues: inclusion of doctors¡¯ payment separating from hospital payment together with publicity of the institutions and the role of private health insurances and baseline coverage of DRG and (2) economical homogeneity related issues: use of claims data not cost-accounting data. The Korean DRG is probably biased from usual DRG principles, and more scientific studies on policies and healthcare system related to DRG are needed. For the full introduction of DRG as payment system, consensus on future systemic changes is desperately needed among the government, citizen, and doctors.
